Transcutaneous Electrical Nerve Stimulation (TENS) at Acupuncture Points - Why not?

TENS and acupuncture/Acupressure do not have the same origin in the development process. In the "TENS and Acupuncture: Understand the Relevance" article, I have a general overview of this issue.

Research on TENS applied in acupuncture points. What do they say?

1. TENS applies Acupuncture theory

Acupuncture is a popular treatment for pain management, but as a time consuming treatment with only temporary effects, has efficiency implications for NHS services. The potential to use TENS over acupuncture points as a non-invasive alternative stimulation, suitable for self-application, has been suggested yet received limited attention in the published research.


Transcutaneous electrical nerve stimulation (TENS) at acupuncture points for the management of chronic pain: a narrative review:

Purpose: This review explores the physiological mechanisms supporting this treatment modality, the clinical efficacy of acupuncture and TENS and the clinical evidence for TENS applied to acupuncture points in order to assess the potential for clinical use and how it should be applied.
Methods/analysis: A literature search was conducted using Medline, EMBASE, CINAHL, AMED and the Cochrane Database for work relating to the physiological mechanisms and clinical effectiveness of acupuncture, TENS and TENS applied to acupuncture points. Details of the reported physiological mechanisms relating to all modalities were extracted and compiled to produce a narrative synthesis of the similarities and differences. Data was extracted from the clinical evidence relating to the treatment dose parameters including frequency, intensity, treatment duration and stimulation location in order to identify themes and patterns, which was used to produce a narrative analysis and review. The quality of clinical evidence was assessed using the PRISMA checklist for systematic reviews and the Downs & Black tool for clinical trials.
Results: Despite similar physiological mechanisms, acupuncture and TENS are often applied with different treatment protocols and their clinical efficacy has been conflicting in the literature. The existing clinical evidence for TENS applied over acupuncture points for chronic pain has generally positive outcomes but is of low quality. Inadequate or excessive treatment duration may be a factor in the efficacy of acupuncture, TENS and TENS applied to acupuncture points for chronic pain. All three may perform best when used for the same optimal treatment durations.
Discussion and conclusions: Applying TENS in the same typical treatment protocol used in acupuncture treatments may be as effective as invasive acupuncture in managing chronic pain. A good quality clinical trial is required to verify the findings of this review.
Impact and implications: The results of this review have implications for the current guidelines on the application of TENS machines and identifies a potential method of maintaining the effects of acupuncture for the long-term in chronic pain patients, thus reducing the demand on NHS services and promoting self-management.

2. TENS applied according to Acupressure/Reflexology 

The term Acu-TENS used to describe TENS applies the method of acupressure therapy. Acu-TENS is not a coincidence.

Conclusion
Contrary to Placebo-TENS, our findings showed a statistically significant enhancement of postexercise FEV1 and prolongation of sub-maximal exercise duration after administration of Acu-TENS in subjects with normal health. In other words, Acu-TENS, a noninvasive modality, is able to attenuate the drop in FEV1 and enhance the respiratory response to exercise. Our study suggests that Acu-TENS has the potential to improve the postexercise FEV1 and may have a role in extending the duration of exercise training.
